Preparing For Your Stay

Pre-admission Phone Interview

Prior to your admission to Holy Family Hospital’s Methuen or Haverhill campus, you will receive a call from a patient registration representative who will gather information for your medical records and answer any general questions that you may have. If you have not heard from us two days before your admission, please call the patient registration office at the campus where you will be receiving your care.

What to Bring

Most of what you need will be provided for you during your hospital stay, but here are some items you don’t want to forget:

  • Medication list

  • Health Care Proxy*

  • Health insurance cards and picture identification

  • Dentures

  • Eyeglasses or contact lenses

  • Hearing aids

  • Pajamas/robe

  • Slippers

  • Personal toiletries

What to Leave at Home

Please leave money, jewelry, credit cards and other valuables at home as Holy Family Hospital cannot be responsible for their safekeeping. If you have any other essential personal items such as dentures or eyeglasses, be sure to inform us about them when you are admitted and we highly recommend that these items be clearly marked with your first and last name.

We request that you leave electrical appliances or anything that requires the use of wall outlets at home. This is for your safety and is in compliance with hospital safety rules. However, you may use battery-operated items such as a razor, a small radio with earphones or a powered toothbrush, as long as these items do not require recharging.

Holy Family Hospital cannot be responsible for replacement of personal belongings.

*Health Care Proxy

The situation may arise when you are unable to make your own health care decisions, or are unable to take part in planning your medical care because of an illness or injury. Holy Family Hospital encourages patients who are 18 or older to complete a health care proxy form that allows you to designate a relative, friend or other person to make these decisions on your behalf. Massachusetts law requires that all patients be given the opportunity to complete a health care proxy form.

Patient registration will provide you with the hospital's health care proxy brochure upon your admission. If you would like to review a brochure prior to your admission, call the patient advocate in Methuen at 978-687-0156, ext. 2399 or dial 2399 from the bedside phone and in Haverhill at 978-521-8540 or dial 8540 from the bedside phone. We encourage you to discuss this issue with your physician, nurse, social worker, and closest relatives or friends.

When You Arrive

Parking is free of charge at both of Holy Family Hospital’s campuses.  

At Holy Family Hospital in Methuen you may park in the lots in front or in back of the hospital. Visitors may also park in the lot behind the acute care facility (Irving E. Rogers Building).

In Haverhill, parking facilities are located at the Main Entrance off Katsaros Drive and at the back entrance off Lincoln Ave. Please do not park in the Merrimack Valley Medical Center (the brick building) parking lot located directly across the street from the Merrimack Medical Center Office Building. This space is not hospital property and violators will be towed.

We ask that you observe parking restrictions for the handicapped. Patients may also be dropped at the main entrance using the circular driveway. During construction and events, please follow parking and entrance instructions for your safety and the safety of everyone. If you have any questions, please ask one of our staff members.
